Benign prostatic hyperplasia (BPH) is the non-cancerous enlargement of the prostate gland that occurs with age in most men, driven by the ongoing conversion of testosterone to dihydrotestosterone (DHT) by the enzyme 5-alpha reductase within prostate tissue. DHT is a far more potent androgen receptor stimulant than testosterone and drives progressive proliferation of stromal and glandular cells, particularly in the transition zone of the prostate that surrounds the urethra. As this tissue enlarges, it compresses the urethra and irritates the bladder neck, producing the characteristic lower urinary tract symptoms: weak stream, hesitancy, incomplete emptying, and — because the partially obstructed bladder becomes irritable and overactive — urinary frequency and nocturia.
