Ear Infection

Ear infections (otitis media) are infections of the middle ear — the air-filled space behind the eardrum (tympanic membrane) — and are the most common reason for antibiotic prescriptions in children globally. Otitis media typically follows an upper respiratory tract viral infection that causes eustachian tube dysfunction: the eustachian tube normally ventilates the middle ear, equilibrates pressure, and drains secretions into the nasopharynx. Viral nasopharyngeal infection causes mucosal oedema and lymphoid hyperplasia that obstructs the eustachian tube orifice, creating negative middle ear pressure and accumulation of sterile secretions (otitis media with effusion — 'glue ear') that subsequently become infected by bacteria ascending from the nasopharynx. Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis are the dominant bacterial pathogens. Symptoms include ear pain (otalgia), fever, irritability (infants pulling at their ear), decreased hearing, and sometimes tympanic membrane perforation with purulent discharge — which paradoxically relieves pain as pressure equalises. Chronic otitis media with effusion causes persistent hearing loss that can impair speech and language development in young children. Eustachian tube dysfunction is worsened by: adenoid hypertrophy (blocking the eustachian tube orifice), passive cigarette smoke exposure (damages mucociliary clearance), dairy consumption (promotes lymphoid hyperplasia in susceptible children), and attendance at group childcare. Xylitol (from birch) inhibits pneumococcal colonisation of the nasopharynx — clinical evidence for reducing ear infection frequency. Elderberry and zinc reduce viral upper respiratory infections that precede AOM. Naturopathic warm olive oil drops provide symptomatic relief for pain. Vitamin D deficiency increases susceptibility.
