Sunburn in children represents ultraviolet radiation (UVR) — predominantly UVB (290–320 nm) — induced acute phototoxic injury to the skin, with childhood sunburns carrying substantially higher lifetime melanoma risk than adult sunburns due to the longer timeframe available for UV-induced DNA mutation accumulation and clonal expansion. UVB photons are absorbed by pyrimidine bases in DNA, creating pyrimidine dimers — specifically cyclobutane pyrimidine dimers (CPDs) and 6-4 photoproducts — at a rate that overwhelms nucleotide excision repair when UV exposure is intense. Unrepaired CPDs in p53 and BRAF are the signature mutations in UV-induced melanoma. The inflammatory cascade producing the visible, painful sunburn develops over 6–24 hours post-exposure: damaged keratinocytes release pro-inflammatory cytokines (IL-1α, IL-6, TNF-α, PGE2), Langerhans cell migration increases, and mast cell histamine release drives vasodilation and oedema — creating the erythema, warmth, pain, and oedema of acute sunburn. Children's skin has lower melanin density and less well-developed photoprotective mechanisms than adult skin, increasing UVB penetration per unit dose. Sunburn first and second degree (blistering) in very young children requires medical attention due to fluid loss and infection risk.
