Oral Thrush

Oral thrush (oropharyngeal candidiasis) is a mucosal fungal infection caused by Candida albicans — a commensal yeast present in the oral cavity of 30–45% of healthy individuals — which transitions from commensal to pathogen when the host mucosal immune defence is disrupted. In neonates and young infants, immune immaturity (particularly naive T-cell responses and reduced salivary IgA) combined with immature mucosa creates vulnerability; neonatal oral thrush affects 2–5% of newborns, commonly acquired during vaginal delivery (maternal vaginal Candida colonisation) or from nipple colonisation during breastfeeding. C. albicans in the hyphal (filamentous) growth phase is the pathogenic form: hyphae physically penetrate the mucosal epithelium expressing specific adhesins (Hwp1, Als family proteins) that bind host cell proteins, and produce secreted aspartyl proteases (SAPs) that degrade host proteins and evade immune recognition. The hallmark white plaques — resembling cottage cheese or milk curds on the oral mucosa, tongue, and palate — consist of fungal pseudomembrane (hyphae, shed epithelial cells, and fibrin); unlike milk curds, they bleed when scraped off. Concurrent maternal Candida nipple infection creates a painful feeding cycle perpetuating re-infection.
