Laryngitis
Laryngitis is inflammation of the larynx — the voice box housing the vocal cords — characterised by hoarseness, dysphonia (altered voice quality), loss of voice, sore throat, dry or tickling cough, and difficulty swallowing. The vocal cords (true vocal folds) are paired muscular folds of mucosa within the larynx that vibrate at precise frequencies to produce voice; swelling and oedema of the vocal cord mucosa from inflammation disrupts their vibration, causing the characteristic hoarse or whispery voice. Acute laryngitis is almost exclusively viral — it typically follows an upper respiratory tract infection (rhinovirus, influenza, parainfluenza being most common) that spreads from the nasopharynx to the laryngeal mucosa. Bacterial laryngitis is rare in adults. Epiglottitis — life-threatening bacterial supraglottic inflammation causing severe dysphagia, drooling, 'hot potato' voice, and inspiratory stridor — is an emergency requiring immediate airway management. Chronic laryngitis (persisting >3 weeks) requires investigation for underlying causes: chronic GERD (acid bathing the larynx and causing posterior laryngitis — 'laryngopharyngeal reflux'), smoking, occupational voice overuse (teachers, singers — causing vocal cord nodules), allergic laryngitis from post-nasal drip, and fungal laryngitis in immunocompromised individuals. Voice rest is the most important acute intervention — continued phonation on inflamed vocal cords worsens oedema and risks haemorrhage.
